How do I register?

The normal start date for the Plan will be immediately after the annual health check and vaccination. The Plan becomes active after the practice has received the first monthly payment.

Please call in at the practice to complete a simple registration form, which will require your bank details.

Practice staff will be happy to answer any queries you may have.
